Findings: The Alleged Victim (AV) has a history of falls. AV had 11 falls between August 2021 and December 2021, mostly which resulted in injury to AV. On October 7, 2021, AV fractured his/her left shoulder. No change of condition was completed by the facility at that time. AV suffered 4 falls in December 2021, the facility failed to implement new fall interventions after those falls. On or about December 28, 2021, AV fell again, re-fracturing his/her left shoulder. The faciilty failed to properly care plan to ensure the safety of AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
