Findings: Witness #1 (W1) has a prior history of touching others of the opposite sex and has been on alert for sexual behaviors. On or about January 5, 2022, the Alleged Victim (AV) and W1 were sitting together in the common area watching TV when W1 put his/her hands in AV's pants and was rubbing AV's private area while AV was asleep. The facility failed to have interventions in place to ensure the safety of residents, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
