Findings: The facility failed to appropriately care plan related to the Alleged Victim's (AV) fall history. On or about January 18, 2022, AV was found on the floor after falling out of his/her bed. AV suffered bruises to his/her arms and legs. The facility's failures are a violation of resident rights, are considered neglect of care and constitute abuse.
