Findings: Alleged Victim (AV) has a known history of behaviors. AV has threated harm to staff and some residents. On or about January 29, 2022, staff reported a disorderly contact from AV to law enforcement. On or about January 29, 2022, AV was involved in a resident-to-resident altercation while trying to take out a resident’s catheter. The facility failed to care plan around AV’s known behaviors, which is a violation of resident rights, is neglect of care and constitutes abuse.
