Findings: The Alleged Victim (AV) is care planned to have staff escort him/her to the dining room in his/her wheelchair. On or about February 24, 2022, the AV was walking back to his/her room with another resident and fell. AV's fall resulted in a broken hip. The facility failed to follow the care plan, as they did not bring AV to the dining room in his/her wheelchair or return AV to his/her room in a wheelchair. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
