Findings: The Alleged Victim (AV) relies on the facility for their care needs to include personal hygiene and oral care. Between January 2022, and early March 2022, it was documented that AV was assisted with getting their teeth brushed approximately three (3) times, resulting in a white film over AVs teeth and a loss of AV’s personal dignity. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
