Findings: Alleged Victim (AV) is care planned to received catheter checks 3x per shift. AV has been found with a full catheter bag on multiple occasions. AV’s catheter bags are not being cleaned properly. AV’s catheter bags have been found on the floor, in the shower and in the sink. Staff are not appropriately trained on how to provide catheter care. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
