Findings: Alleged Victim (AV) relies on the facility for his/her care. AV is at increased risk for skin breakdown and other chronic skin conditions which require daily monitoring and treatment. Between January 10th through March 6th, 2022, AV was on alert monitoring for open wound, but the alerts were closed on March 6, 2022. On or about March 24, 2022, AV was taken to the hospital where he/she was found to have state 2 wound on his/her sacral region, inflamed skin in his/her groin area, and swollen/inflamed cellulitis on his/her right leg. The facility failed to provide oversight and monitoring of change of condition, which is a violation of resident rights, is neglect of care and constitutes abuse.
