Findings: Witness 4 (W4) has a known history of violent behaviors towards staff and residents. On or about March 30, 2022, W4 an Alleged Victim (AV), were involved in a resident-to-resident altercation which led AV to sustain a fall and caused him/her an injury to the right scapula and top of the scalp. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
