Findings: Alleged Victim (AV) has a history of falls. On or about March 31, 2022, AV had an unwitnessed fall and was found on the floor. AV sustained abrasions and a skin tear. AV had a bed alarm to alert staff when AV is trying to get out of bed. It was discovered the care plan does not reflect the use of a bed alarm or re-setting the bed alarm after it has been set off. The facility failed to appropriately care plan, which is violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP22-01405 $250.00 fine assessed
