- Findings
- Alleged Victim (AV) has a known history of falls. Between January 5th through March 28th, 2022, AV sustained six (6) falls. AV has had several falls prior to 2022. There have not been interventions provided for several non-injury and injury falls. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
