Findings: The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs, including lack of appropriate interventions which include safety checks, and AV's known unreliable use of the call system. AV experienced approximately six (6) unwitnessed injury and non-injury falls in the month of April 2022. AV injuries included broken finger, re-open of prior skin tear, abrasions to knees, and laceration to scalp. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
