Findings: The facility failed to provide appropriate services according to Alleged Victim (AV)'s needs. The Alleged Victim (AV) was care planned for a weekly shower and assisted with compression stockings twice daily. On or about, March 17, 2022, the Alleged Victim (AV) was found to have a laceration and infection to his/her toe, which resulted in unreasonable discomfort. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
