Findings: Witness #1 (W1) has a history of altercations and is care planned for staff to engage W1 in conversation or offer a snack when W1 is showing agitation. On or about April 11, 2022, the Alleged Victim (AV) approached W1 multiple times, staff tried to redirect AV but were unsuccessful, AV approached W1 again and W1 pushed AV creating a skin tear on AV's hand. The facility's failure to follow the care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
