Findings: The facility failed to implement interventions and appropriately care plan related to AV’s history of falls. The failure resulted in AV experiencing multiple falls from approximately January 14, 2022, and March 11, 2022. As a result of the last fall, AV was sent to the hospital and surgical intervention was required. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
