On or about May 29, 2022, AV, and Witness 1 (W1) were in a shared common area, and AV was knocked to the ground by W1, which caused an abrasion to AV’s elbow. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
