Findings: Alleged Victim (AV) is care planned to be a two-person assist. On or about June 4, 2022, a staff member was helping AV change when AV stumbled backwards and fell. On June 5, 2022, AV was feeling pain and was given Tylenol, but the pain continued. The facility failed to follow the care plan around AV being a two-person assist, which is a violation of resident rights, is neglect of care and constitutes abuse.
