- Findings
- The facility failed to follow Witness 1's care plan to monitor and redirect according to his/her known behaviors. The failure resulted in inappropriate sexual contact with AV, on or about April 25, 2022, causing unreasonable discomfort and a loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
