Findings: On or about June 19, 2022, the Alleged Victim (AV) suffered a fall in his/her room while trying to transfer from the bathroom back to his/her wheelchair. AV was left standing in the bathroom alone by staff. AV is known not to use his/her call light when he/she needs help. AV fell and suffered a cervical spine fracture and a scalp laceration, requiring 15 stitches. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
