- Findings
- Between approximately July 3, 2022, and July 7, 2022, Alleged Perpetrator 2 (AP2) did not ensure the Alleged Victim’s (AV) medication was administered as ordered, which resulted in the Alleged Victim (AV) experiencing unreasonable discomfort and loss of personal dignity. AP2's actions are considered neglect and constitutes abuse. The facility failed to provide a safe environment, which violates Oregon Administrative Rules.
