Findings: Alleged Victim (AV) relies on the facility for his/her care. AV was a sliding scale insulin prior to being admitted to the facility. AV’s MD ordered sliding scale insulin on or about March 29, 2022. The facility agreed to provide assistance with medication and treatment requiring a registered nurse training and supervision, including blood sugar testing and insulin. However, the facility refused to provide sliding scale insulin for AV, resulting in AV’s diabetes being out of control; AV being sent to the hospitals on multiple occasions, his/her blood sugar was out of control, his/her A1C was high, and his/her weight was off. The facility failed to provide service to AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
