Findings: The facility failed to provide a safe environment. According to an investigation, Witness 4 (W4) has a history of behaviors and resident-to-resident altercations. On or about July 16, 2022, W4 punched the Alleged Victim (AV) on his/her forearm while they were sitting at the table in the dining room, which resulted in unreasonable discomfort and loss of personal dignity. The facility’s failure is a violation of resident rights, is neglect of care and constitutes abuse.
