Findings: Witness #1 (W1) is known to have behaviors and is care planned to be assisted or redirected when having behaviors. On or about July 26, 2022, W1 was agitated, staff were assisting other residents, W1 wandered down the hallway and engaged in an altercation with the Alleged Victim (AV), causing injury to AV's nose, a skin tear and a laceration to the back of AV's head. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
