Findings: Alleged Victim (AV) relies on the facility for his/her care. AV is known to have swelling in his/her legs and staff have been directed to ensure AV is in bed and not sleeping in his/her recliner, however, there is nothing noted in AV's care plan regarding this issue. On multiple occasions, AV was found with socks that were too tight and his/her legs were swollen. On or about April 18th, 21st, 25th, and 27th, 2022, AV had been left in a recliner overnight, causing swelling to AV’s feet, which is a risk of serious harm. The facility failed to properly care plan around AV’s swelling of the feet, which is a violation of resident rights, is neglect of care and constitutes abuse.
