Findings: The facility failed to provide a safe environment. The Alleged Victim (AV) and Witness 1 (W1) have known behaviors. W1 is care planned for staff to be aware of W1's proximity to other residents when W1 is in a common area. On or about, August 10, 2022, the AV and W1 had a resident-to-resident altercation when AV and W1 were left alone in a common area, which resulted in AV suffering from a bloody nose and a scratch on his/her arm. The facility’s failure is a violation of resident rights, is neglect of care and constitutes abuse.
