Findings: Alleged Victim (AV) relies on the facility for h/h care. The facility failed to provide a safe medication administration system to ensure AV medications were ordered timely and given per physician orders. Between approximately April 4, 2022 and July 18, 2022, there are multiple instances documented where AV’s medications were not onsite and therefore not given to AV which led to AV exhibiting multiple symptoms, including increased agitation. The facility failed to provide a safe medication administration system, which is violation of resident’s rights, is considered neglect of care and constitutes abuse.
