Findings: The Alleged Victim (AV) is a known fall risk. AV suffered falls on August 22, 2022 and August 23, 2022. AV's service plan had not been updated regarding falls since June 3, 2021. AV fell again on August 30, 2022 and was unable to bear weight. The facility's failure to properly care plan to reduce the risk of falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
