Findings: The Alleged Victim (AV) relies on the facility for his/her care. On or about August 3, 2022, the Alleged Perpetrator #2 (AP2) discovered a bruise on AV's arm. AP2 noted the bruise in AV’s progress notes but failed to report the bruise to anyone else to rule out abuse. AP2 did not follow abuse reporting guidelines which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to ensure the residents rights/treatment was being followed, which is a violation of Oregon Administrative rules.
