Findings: Alleged Victim (AV) is care planned as a two-person full assist. On or about September 16, 2022, Alleged Perpetrator 2 (AP2) attempted to transfer AV without assistance from another staff member. As a result, AV fell during the process of being transferred. AP2 failed to follow the care plan, which is neglect of care and constitutes abuse. The facility failed to assure the care plan was being followed, which is a violation of Oregon Administrative Rules.
