Findings: Between July 1, 2022 and September 7, 2022, the Alleged Victim (AV) suffered approximately 14 falls, with and without injury. Some interventions were put into place, however, AV continued to fall before some interventions could be put into place. AV suffered pain and unreasonable discomfort. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
