Findings: The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs, relating to his/her change of condition, number of falls with lack of appropriate interventions. Based on the investigation, AV experienced approximately five falls, between July 12, 2022, and October 14, 2022. On or about October 19, 2022, AV experienced another fall, which resulted in AV being sent to the hospital for head pain and unreasonable discomfort. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
