Findings: The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs, relating to, number of falls with lack of appropriate interventions. Based on an investigation, AV experienced multiple falls, between August 16, 2022, and October 14, 2022. On or about October 19, 2022, AV experienced another fall where AV hit his/her head, resulting in unreasonable discomfort. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
