Findings: The Alleged Victim (AV) has suffered 11 falls between July 18, 2022 and October 2, 2022, suffering lacerations, bumps, bruises and skin tears. AV was to be wearing shoes, and during a few of the falls, was found without shoes. AV was also to be supervised and staff were to anticipate AV's needs when he/she is in the common area, however, AV had 3 unwitnessed falls in the common areas. The facility's failure to follow AV's care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
