Findings: On or about October 27, 2022, the Alleged Victim (AV) was found on the floor from an unwitnessed fall. AV suffered a head laceration requiring staples. AV was wearing improper footwear, which lead to his/her fall. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
