Findings: On or about December 11, 2022, the Alleged Victim (AV) complained to staff that Alleged Perpetrator #2 (A2, unknown) ran over his/her foot with the wheelchair. AV did not have his/her feet in footrests, as his/her footrests are in the closet as they do not fit on AV's wheelchair. AV suffered a bruise on his/her foot due to it being ran over by AP2. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility's failure is a violation of Oregon Administrative Rules.
