Findings: The facility failed to provide appropriate services according to Alleged Victim (AV)'s needs, number of falls with lack of appropriate interventions. Based on an investigation, AV experienced approximately five falls from approximately November 5, 2022, and January 22, 2023, which resulted in repeated unreasonable discomfort because of some head injuries. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
