Findings: Alleged Victim (AV) requires 24/7 care by the facility. On or about February 20, 2023, AV followed another resident’s family member out of the facility. Visitors of the facility reported a person matching AV’s description walking down the street. AV was located by the police 30 minutes after leaving the facility and had crossed a street, which exposed AV to potential risk of harm. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
