Findings: The Alleged Victim (AV) was a known fall risk and suffered multiple, frequent falls with and without injury over approximately 45 days. On or about February 15, 2023, AV suffered a fall, resulting in a femur fracture. On February 21, 2023, AV was placed on hospice and diagnosed with a left hip fracture. The facility's failure to properly care plan regarding AV's unsafe behaviors which lead to AV's increased falls, is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-00763 $2500.00 fine assessed
