Findings: The facility failed to appropriately care plan and provide supervision according to the AV's care needs and change in condition. The failure resulted in AV experiencing an unwitnessed fall and laid on the floor in pain for an undetermined period of time due to not being check on for approximately four hours, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
