Findings: On or about March 3, 2023, the Alleged Victim (AV) was exhibiting behaviors of yelling out, which was not addressed by staff. Witness 1 (W1) has a history of aggravation and aggressive behaviors, and is to be redirected from other residents to avoid altercations. The facility and staff failed to provide adequate supervision according to the needs of the residents, resulting in a physical altercation between W1 and AV. The Alleged Perpetrator 2 (AP2) failed to follow W1's care plan to redirect him/her from other residents to prevent further injury, and left the residents unattended during a physical altercation. The facility and AP2's actions are a violation of resident rights, considered neglect of care which constitutes abuse.
