Findings: The facility failed to follow Witness 1's (W1) care plan according to his/her need for monitoring and redirecting due to ongoing aggression and altercations. W1 wandered into another Pod of the facility without staff knowledge and got into a physical altercation with the Alleged Victim (AV), pushing AV to the ground, and two other residents had to intervene and redirect W1. The facility's failure caused unreasonable discomfort to AV, and placed other residents at risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-01225 $375.00 fine assessed
