Findings: On or about March 29, 2023, the Alleged Victim (AV) suffered a fall in his/her apartment resulting in a skin tear to his/her elbow. AV had previously suffered falls on March 22, 2023 and March 25, 2023. From the March 22, 2023 fall, AV was sent to the hospital and diagnosed with a UTI. The facility failed to have the facility RN to intervene and perform a change of condition assessment which would have assured AV was being care planned appropriately as it related to his/her falls. The facility's failure to properly care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP23-00461 $188.00 fine assessed
