Findings: According to the documentation, the Alleged Perpetrator (AP2) failed to follow the service plan for the Alleged Victim (AV) when they display signs of irritation or aggression. The failure resulted in the AV punching AP2 and AP2 punching the AV back in the chest, which is a violation of resident rights and is considered physical abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
