Findings: According to the documentation, the facility failed to implement interventions for the Alleged Victim’s (AV) fall history. The failure resulted in the AV suffering numerous falls with injury, was not able to ambulate by themselves anymore due to fall with injury and was later admitted to hospice, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
