Findings: Under the prior ownership: The facility failed failed to appropriately care plan related to the Alleged Victim's known behaviors. AP2 failed to provide appropriate supervision according to the AV's care plan. The failures resulted in a physical altercation causing unreasonable discomfort to AV who was slapped in the face, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
