Findings: The facility failed to provide adequate supervision to Alleged Victim (AV), which resulted in AV suffering multiple falls with injuries while self-propelling in his/her wheelchair. Based on facility documentation and interviews, at time of investigation AV's care plan did not reflect AV's current care needs related to ambulation which placed AV a risk of more falls. The facility's failure to properly care plan for AV's fall risk is a violation of resident rights, is considered neglect of care and constitutes abuse.
