Findings: On or about June 13, 2023, Witness 1 (W1) punched Alleged Victim (AV) twice in the upper left arm causing AV unreasonable discomfort. According to facility documentation and interviews, the facility failed to implement appropriate interventions and failed to appropriately monitor W1 according to his/her known behavior and prior altercations. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-01241 $375.00 fine assessed
