Findings: On or about June 13, 2023, the facility failed to follow the Alleged Victim (AV) care planned for two-person transfers with toileting assistance. AV was assisted with one person while toileting instead of two, which resulted in AV falling suffering pain, and unreasonable discomfort. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
