Findings: Alleged Victim (AV) relies on the facility for assistance with transfers and the use of a gait belt during transfer. On or about June 27, 2023, the Alleged Perpetrator 2 (AP2) did not follow AV's care plan and transferred AV without the use of a gait belt resulting in AV being placed on his/her bed in a rough manner. As a result, AV expressed fear of AP2 and did not want AP2 in his/her room. AP2's actions are considered neglect and constitutes abuse. The facility failed to ensure care plans were followed which is a violation of Oregon Administrative Rules.
Sanction: ALFCP23-00683 $500.00 fine assessed
