Findings: The Alleged Victim (AV) has a history of exit seeking and elopement. On or about July 4, 2023, AV was left sitting outside on a bench. AV followed staff from the secured area to the front lobby and was found across the street at a shopping mall, placing AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
